Letter to my School Requesting Complete Allergen Lists?

Context: My high school uses a site called “MealViewer” to provide the school lunch menus to students. It has very limited allergen options (as follows): Milk, egg, peanut, tree nuts, fish, shellfish, soy, wheat, and dairy. That’s all well and fine, but I have 14 food allergies: Peanuts, soy protein (can have all other parts of soy but no protein or whole soybeans), shrimp, chickpeas, black-eyed peas, pea protein, pumpkin seeds, sunflower seeds, hazelnuts, walnuts, cashews, pecans, lentils, and bananas.
I (15m) eat a lot, because I am a teenage boy. I bring a lunch from home, but it’s usually not enough food, so I resort to getting a school lunch as well. There are a handful of meals I’ve been eating since elementary school, but if my mom can’t bring me extra food (I pack snacks, but they don’t tend to make it to lunch and I don’t have space in my bag for more), I would be risking my life to get a school lunch, especially because they serve things that contain soy protein (which the MealViewer doesn’t always alert me to despite selecting soy as an allergen), and peas, which it doesn’t alert me to, among other things. I need to send an email to the principal to see if they can provide a full ingredient list to me. I can’t find any templates online, as I’m bad at sending emails without one.
Does anyone have a template they could share?
