2023.03.28 20:01 no_eht_no Would you change from AG ACNP to FNP?

Hey all! I am finishing up my first year in a AGACNP MSN program. I've been in the ICU for 2 years, and love acute care. This is what I want to do which is why I applied to the acute care program to begin with. However I do not know any acute care practitioners. In my hospital system everybody that works acute care has their FNP. I also hear that's actually not the norm in most places and that a lot of hospital systems require you to have an acute care np to work in the ICU.
I'm hearing a lot of pushback that I should switch to FNP so not to limit my future career prospects, such as working in urgent care, or the ER. I do also hope to travel in the future with like locum jobs and such.
Apparently My advisor left at school and I have nobody to speak to at the school about this, but I did request to change to the FNP program with the plan to finish acute care afterwards, but as of right now there are no spots for me to switch.
So what would you do? If you would only end up getting one degree is in better to just get the FNP? Try to do a secondary post master certificate? Just do what you love in the first place? Any advice appreciated!

Homecoming. After 12 years. Arkady Lobanov was not a sentimental person but memories of his childhood came flooding in as he got closer to his destination - St. Petersburg.
“You know, my father would always fondly recall his trips to St. Petersburg and he would always come home with gifts for me and my sister.” James Pierre, Arkady’s friend remarked
“I did accompany him when I was 9 years old to meet his great-aunt Sofia Belinsky, but I hardly remember anything from the trip except that I fell of a horse and got a hiding from mother.” He continued as he surveyed the chequered board in front of them.
“Ah! I see that you almost had me distracted Arkady. But trust me, I am getting better at this” James moved his bishop to counter Arkady’s attack.
After a brief pause Arkady smiled and replied “I agree that you are getting better. And yet, you completely forgot about my knight who is in position to take your queen and with that this game”.
The sun was just sinking in the horizon and one of the train crew members went around the carriage lighting up the candles. James shook his head and looked distraught “I thought I almost had this. Guess you are a genius after all!”
“Or that I have had plenty of time to practice with my father” Arkady replied smartly as he rose from his seat.

Dinner was soon served and over the main course of roasted beef served with potatoes and mashed turnips, the men discussed their impeding trip.
“I do have to warn you, the Russian parties are even more boring than the French ones. Worse there aren’t enough girls our age who would be in attendance either. It will be old hairy warlocks, members of the Duma boring everyone with stories of legislative assemblies and quite a few married ladies with saggy tits fawning all over you to start a summer affair”
Arkady smiled behind his glass of wine. “It can’t be that bad. Personally, I am very excited to visit after 12 years.”
“While I understand your enthusiasm, I am afraid I can’t mirror that sentiment. Believe me, I would skip this trip entirely if I could” James sounded exasperated.
“I am sure not having to wake up for the lectures early morning would be a welcome change. Plus, we could hunt for game and go horse riding. Camp in the forest. As a matter of fact, an occasional liaison with a married woman wouldn’t be so bad do you think? I mean they would probably have more experience won’t they?” Arkady probed.
“Hah! To think that Arkady, the son of pious and celibate Mathematics Professor Mikhail Lobanov is interested in steamy liaisons with married woman of the Russian Aristocracy!” mocked James. “Alright Arkady. I see that you are a man with a mission. I don’t hope to dissuade you nor rain on your excitement. Let’s toast to a summer of adventures” James raised his glass. Arkady soon followed suit, a flicker of shadow crossing his face at the mention of the word mission.

Five carriages away, 2 men played cards while another was in deep conversation with a passenger. The three of them were travelling together. The train horn signalled the approaching station and one of the men rose from their seat. Wordlessly, he picked up his luggage and bid his goodbyes to the other man. The train slowed down and stopped at the small village town – Gatchina where this man got off the train and hurried off the empty station. This man who went by the name of Vadim, would band up with the Bolsheviks and gather resources and men for their eventual mission while the other two men would work in the shadows for Arkady.

By 8 o clock, they had reached St. Petersburg and James and Arkady were met on the bustling station by Yuri Nosov – a butler arranged by James’ father and were escorted to their horse carriage. St. Petersburg looked bright and crowded and dirty and they were soon brought to the mansion were they would be staying the night arranged once again by James’ father.
“Mr. Ivanovich apologises to not being able to receive you personally as he was called to the court for an urgent matter. You have.. heard the news of the unrest in the capital I am sure? These are dangerous times.” The wife of Mr. Peter welcomed the young men, smiling and eyeing them surreptitiously.
“Seems like Mrs. Ivanovich might have taken a shine for you Arkady” James teased him teasingly as they settled in their rooms.
“I did warn you about old married ladies here”
“I just think she was nervous welcoming us James. Your father is obviously important to their business and they didn’t want to offend us”
“My naïve friend, that’s not all of it, I am sure – ”
A knock on the door interrupted their conversation and Yuri handed over a letter to James.
James tore open the letter and reading its contents, smirked and yawned “Here is the invitation for our first party. The Khilakov’s. Father warned me that they would court us. Lord Basil has recently found himself out of favor with the Tsar as his business venture in the east is overrun because of the Japanese invasion of Manchuria. But I am told his wife has an appetite for young men so this could be an interesting introduction to the debauchery of the Russian aristocracy for you my friend.”
James dropped down on his bed as he handed Arkady the invitation. Arkady’s heart was thumping against his chest as he read the contents of the invitation. It was all coming together as planned and yet he found it moving faster than he thought. He stayed awake late into the night, as dark thoughts about the future plagued him even as his friend snored beside him without a care of what was to come.

Lo and behold, I’ve been working 5 days a week through every possible hour I gave. I also haven’t even signed a W-4, despite receiving my first paycheck. When I went to my boss to fill one out, she said that we still needed to do my orientation with another new employee and that I could fill it out whenever we found the time to do that. Feeling frustrated, and a little sussed.

2023.03.28 19:13 sultamicillyn I work overtime because I care, but I'm made to believe that's because I'm stupid

I (30F) am a healthcare worker in a public hospital. Now, I'm honestly not the fastest thinker and I know I am slow with my work. I like spending a little extra time with the patients to chat with them, but obviously there's more than one, and all the extra time I spend adds up. So I've come up with a work schedule I'm comfortable with - in the morning, I talk to the patients, sort out the list of stuff that needs immediate attention, update patient families about the latest plans etc. In the evening, I go through the summary for each patient and sort of try to see whether I've missed anything or not. That means on most days, I'll go home an hour or two later. I'm not paid overtime, but I don't mind most of the time, because I don't like rushing. Trust me, I've tried, but it just makes me feel more stressed out because I really like to do things at my own pace, and it resulted in things being overlooked for my patients and it always ends up with me asking myself "have I done my best?" and nothing feels worse than when the answer is a clear "no". Also, occasionally patient families decide to come well after work hours and believe me when I say the ppl working at night literally don't know what's going on. Their job is to make sure everyone stays alive till morning, not to answer a "concerned" relative's questions because they somehow aren't "concerned" enough to come or at least call the hospital during the day.
Anyhow, it soon became common knowledge in my department that I always go home late. I don't make a fuss about it, and ppl generally leave me alone to do my thing, so I'm good. Recently, though, ppl have been constantly nagging me to go home on time, to the point they actually pack up my stuff and kick me off the ward. The few times this happened, I overlooked things that led to patients not getting the treatment that they should have received on time. Usually, it's minor stuff that really didn't matter if it was done a day or two late. A few times, though, it led to rather serious issues being overlooked and patient actually deteriorated because of it.
I've also been spammed with messages and pictures telling how, if I work overtime, I'm essentially an inefficient worker, and that if I get sick, my bosses won't come to my rescue, whereas my friends and family will, so I should prioritise the latter more; the government won't appreciate my efforts and won't pay me extra; I'm just being exploited... Yada, yada. The thing is, in my department at least, my "bosses" (senior doctors) have proven time and again that if one of us got sick, they would go above and beyond to make sure we get extra attention. They are honestly the reason I'm even as dedicated as I am to begin with. But even if they weren't, how would you feel if you or your loved one was admitted and everyone in the hospital just treated you like a job that they wanted done so they can go home on time? Sure, the government doesn't appreciate my efforts, but it matters to me that patients appreciate me. Do I wish I get paid extra? Yes. Do I want to compromise patient care because I'm being paid less per hour than a Starbucks part-timer? No. Honestly, being half-assed about patient care stresses me out more than working overtime. I've literally tried it for two weeks and I nearly broke down in tears from the constant cycle of "let's do this tomorrow" only for tomorrow to bring more work so I never got around to settling the "minor" details and ultimately patients started collapsing everywhere like flies because they're being overlooked.
In short, I'm very, very annoyed. I don't expect ppl to give me awards for working extra hard, but I really don't think I deserve to be put down for choosing to work overtime. Am I slow? Yes, I am, and I know I'm slow. But do I deserve to be constantly hounded by all these posts about how working overtime is just me being (in simpler terms) stupid? I don't think so. I wish ppl who call ppl who work overtime stupid would think about how they would feel if their doctor basically left them to gasp for air because it's time for them to go home so they decided the patients' problems are best left till tomorrow. Or if their nurse let them sit in soiled sheets for another two hours because it's almost time for them to go home. Or if the ambulance driver decided to let them wait until the next shift before transferring to the nearest center them with the urgent medical treatment they needed. I mean, sending a patient to a specialist center that is two hours away but my shift ends in three hours? Let them wait for the next shift, because a round trip would be four hours, making the me go home late by an hour. Why bother, right? It's not my friends or family at stake here, and if I ever get sick, it's not like this dude is going to be there for me. Why stick my neck out and go the extra mile? I wish ppl reading this realise how absurd and outrageous this sounds. Yes, it's the government's job to make sure that we, as healthcare workers get paid appropriately. But should patients suffer because we are at war with the crappy government? No. Wars should not involve innocents.
TL;DR: I am slow and frequently work overtime to compensate, but I feel justified spending extra time on patient care and hate it that ppl are belittling me for my efforts.

I was so hopeful when I started as an NP to have better control of patient outcomes and patient care. I was feeling sooo good knowing I can finally deliver good care to people, and I could know I was making a difference. I work in urgent care and couldn’t feel any more questionable about my career choice.
The disrespect from patients is enamoring. So many people come in demanding “z-pack” for symptoms that started yesterday..they do not care or listen to your clinical professional advice..all parents seem to want is school notes and treat you like a school note machine. One parent today blatantly ignored my advisory and said “I just need a school note”. The people that come in demanding things..I wasn’t fully prepared for this.
I feel so disrespected as a professional, and it’s crushing my spirit. I used to work in derm (dream job, I know) for a year before I had to leave due to the physician literally running me into the ground. I’ve been an NP for almost 3 years now, a nurse for 10.
Has anyone considered leaving? If you did leave, what did you do? Willing to think about anything at this point. Not wanting to work as a staff nurse, I draw the line there. Just curious as to how many of you out there feel the same way.

So last week I noticed my throat was a little sore. Couldn’t see anything irritated looking in the back on my throat. I used a tongue compressor and I could see some white spots right past my tongue going down my throat. Went to Urgent Care and got a Strep A swab test on Sunday (2 days ago) and it was positive. So then I was started on the antibiotic, CEPHALEXIN. Throat seems to be getting a little better as I’m 2 days into the antibiotics….. however, I am extremely fatigued! Also to mention I swabbed positive for the Flu around March 9th or so
As in anytime I’m not doing anything and sit down, immediately almost start nodding off and I’ll drop my phone out of my hands and everything. It’s bothersome especially at work and is starting to make me think I have something more wrong with me as I do get anxiety. Even if I only nod off for a few seconds I’ll come to feeling disoriented like I took a mid day nap or something.
My question is, can Strep Throat really make you THAT tired and fatigued? It’s been a long time since I’ve had it so I’m not quite sure how exactly it effected me last time

2023.03.28 18:48 meggyrose_ How to get tested for jugular/upper extremity thrombosis?

I have a suspicion I have a clot in my jugular vein or somewhere in my right arm, but I can’t get doctors to take me seriously because my d-dimer is currently negative and my arm isn’t “swollen and red”. Even though online it says clots often don’t have any symptoms.
I tested positive for covid on January 6th and had been having chest pain nonstop (still occasionally get chest pain). I had right-sided neck pain appear February 13th. I didn’t think much of it because I didn’t know you could get clots there. Saw a cardiologist on Feb 16th who ordered a d-dimer test for my chest pain. It was high (1.29). I go to ER, rule out PE. A few days later I develop leg pain, get 2 ultrasounds over the next 2 weeks to rule out clots there. D-dimer rechecked March 1, still high. Still have neck pain and still not making the connection that I could have a jugular clot. March 18th I notice new bulging and visible veins all over my body, and one going across the right side of my chest. Wasn’t too worried about it immediately because lots of covid long-haulers have this. This past weekend I saw a random Facebook post of a girl who posted a picture of a chest vein just like mine and said she had a jugular clot and that is often a symptom. I’m like holy crap I wonder if I have a jugular clot because I’ve been having neck pain and that exact vein on that side is visible!!
I can’t see my PCP until April 5th. I went to urgent care this weekend who said I “slept funny” (I know what that feels like and this ain’t it). Went to ER last night where they ran another d-dimer and it was negative so they refused to do any further testing. I’ve heard SO many doctors say that “if d-dimer is negative it 100% rules out a clot” which makes my blood boil.
Anyways, any advice on how to get tested or studies I can point to that show that d-dimer goes back down after the body gets used to a clot? (I saw somebody state that in this sub once). And what test do I need?? Ultrasound? CT of neck?
I really feel like I have a jugular clot because it’s finally adding up with my d-dimer timeline. I know d-dimer can be positive for a bajillion reasons, but it just makes SO much sense with the timeline. I also was told I have antiphospholipid syndrome (I had high anticardiolipin in blood work a month ago). My neck pain has been on and off, but I read online that clots can have so many different symptoms so idk. I also started feeling it go down my arm this past weekend. The ER doc thinks it’s musculoskeletal but I’m not too convinced.

Hey there. 29f that may possibly have Crohns. I was diagnosed with endometriosis last year with laparoscopic surgery. I suspected endo for years, but also had tons of bowel issues with it. I just thought the bowel issues were exasperated by the endo. Over 6+ months after surgery and still struggling with bowel issues. Constipation alternating with diarrhea, stomach pain, gas, bloating, lots of heartburn, tenesmus, incomplete evacuation, always extremely fatigued and when stool is somewhat solid I struggle to relieve myself (stool gets stuck on the way out). I have had blood only a couple times but not regularly. And it wasn't really that much. I have also lost about 20lbs but that could be from not having the endo inflammation anymore? I also tried a complete elimination diet to try to pinpoint my triggers but couldn't even get to the reintroduction phase because the symptoms never stopped.
I had gastroenterology appointment in early February and after explaining all my symptoms she said she believes it may be Crohns. I never even considered Crohns because in my head my symptoms are not as bad as what other people describe with Crohns. It does affect my life a lot but I always thought people with Crohns are absolutely debilitated?
I have a colonoscopy and endoscopy scheduled but they didn't have anything available until mid May. I did the blood and stool test. My blood test came back normal. But my pancreatic enzymes were low. I got put on Creon for that. I also got a CT scan just to make sure the low enzymes aren't from a blockage. No blockage present and everything looked normal.
With some additional research, I also have seen people mention bouts of erythema nodosum with Crohns. I do recall some years ago having what I thought was spider bites on my shins for several weeks. Went to urgent care when it happened but doctors didn't know what it was. Maybe erythema nodosum?
With all this time in between the colonoscopy I am constantly questioning everything. Maybe I'm making my symptoms out to be more than they are? If blood tests are normal can you still have Crohns? I'm not having the constant blood people often describe. It also seems most people were diagnosed at a much younger age, so can people develop Crohns in their late 20s? I'm also confused on what would cause the low pancreatic enzymes?

26M, height is 5’9, weight is around 300 lbs, no medications, no smoking, I used to have an ingrown hair on my armpit that turned infectious, just started yesterday and it’s in my pubic area, can’t add a photo due to the area it’s in.
I believe I have an ingrown hair in my pubic area and I’ve read that it’ll go away on its own but I also ready that if it hurts it’s infected which mine does. I don’t have insurance though so I’m trying to avoid a costly ER or urgent care visit if possible. Thanks!

Inspired by another post I just saw, I'm making a post to remind myself how I'm feeling now when temptation creeps back.
After weeks of regular drinking, I went about a week without and felt like it was proof that I didn't *really* have an issue. The following week, I drank a bottle or more of wine every night, but convinced myself even that wasn't so terrible. By the weekend, the wheels completely came off. I drank throughout the day on Saturday in a way that could have messed up some creative projects that day (though luckily didn't). I then got dinner and drinks with a friend and instead of going home when they did, I stayed at the bar and got drunker. I have no idea what happened next, until I found myself in urgent care with a cut on my head. I was incredibly rude to the staff, insisting that I was fine and needed to leave (I was scared of the cost), as they put staples in my head. I finally left at 4 in the morning, incredibly ashamed and embarrassed.
I drank again on Sunday and Monday, though not to excess - just to ease the shame and anxiety and hangover. I also spent both days reading this subreddit, listening to interviews about addiction, and finally telling a couple of people that I realized I had a problem. Yesterday, I finished a bottle of wine and went to bed. This morning, I read this subreddit and downloaded a day counter app. I've also started looking for meetings after work today.
I've had the experience many people here have described, of drinking moderately for a time and believing that means I'm okay. Putting this here, so I don't make that mistake again.
